When Conventional Implants Are Not Enough
Being told you are not a candidate for dental implants can be devastating — especially when you have been living with missing teeth, failing dentures, or a compromised smile for years. But what many patients do not realize is that a no from one provider is not necessarily the final answer. Advances in implant surgery over the past two decades have opened the door to solutions that did not exist a generation ago, and NorthStar Implant Dentistry specializes in exactly these cases.
The most common reason patients are told they cannot receive conventional implants is insufficient bone. When teeth are lost, the jawbone begins to resorb within weeks. Over months and years, significant bone volume can be lost. For these patients, standard implant placement is not feasible using conventional approaches — but that does not mean implants are impossible. Below are the six advanced remote anchorage techniques Dr. Kampf offers for patients who have been turned away elsewhere.
Technique 1 of 6
Zygomatic Implants
Zygomatic implants are long titanium implants — typically 35–55mm — that bypass the deficient upper jawbone entirely and anchor instead into the dense bone of the cheekbone (zygoma). Developed in the 1990s and now with decades of clinical evidence, they are one of the most proven solutions for patients with severe upper jaw bone loss. In many cases, a full fixed set of teeth can be delivered the same day as surgery. They may be used singly, in pairs, or in combination with pterygoid or trans-nasal implants for comprehensive full-arch rehabilitation.

Pterygoid Implants
Pterygoid implants are placed at a steep posterior angle, passing through the tuberosity of the upper jaw and anchoring deep into the pterygoid plates — dense bony structures located behind the maxilla that are largely unaffected by the resorption that plagues the ridge. Often used bilaterally in combination with anterior or zygomatic implants, pterygoid implants are a key component of comprehensive full-arch rehabilitation for patients with severely atrophic upper jaws. They eliminate the need for posterior sinus grafting and allow immediate loading in qualifying cases.

Trans-Nasal Implants
Trans-nasal implants are a recently developed technique for the severely atrophic premaxilla — the front portion of the upper jaw — where conventional implants are not feasible. The implant is inserted from the mouth, passes through the nasal cavity at a precise angle, and anchors in 3–5mm of dense bone at the frontal process of the maxilla. Trans-nasal implants are used in combination with zygomatic implants to support a full-arch restoration, reducing or eliminating the need for a quad-zygoma approach. They require careful CBCT-guided surgical planning and are performed only by surgeons with specific expertise in craniofacial anatomy.

Trans-Sinus / Tuberosity Implants
Trans-sinus implants are placed at the ridge crest and angled mesially, passing through the maxillary sinus and anchoring simultaneously in the crestal bone and the nasal wall — achieving double bicortical anchorage. This provides excellent primary stability without requiring zygomatic placement or extensive sinus grafting. A small amount of bone graft material is placed around the exposed portion of the implant within the sinus during the same appointment. Trans-sinus implants are particularly useful in Bedrossian Zone I and II atrophy where the sinus has expanded but some crestal bone remains. They offer a technically demanding but highly effective alternative when anatomy permits.

Sub-Periosteal Implants
Sub-periosteal implants rest directly on the jawbone surface beneath the gum tissue, rather than penetrating the bone. They are custom-fabricated using a 3D CBCT scan — the resulting titanium framework conforms precisely to the ridge for intimate, distributed contact. Abutment posts protrude through the gum to support the prosthesis. Sub-periosteal implants are ideal for patients with extreme bone loss who cannot accommodate any endosseous implant — including zygomatic — and who are not candidates for bone grafting. Modern CBCT-guided fabrication has dramatically improved fit and long-term outcomes for this technique.
Technique 6 of 6
Trans-Mandibular Implants
Trans-mandibular implants are a specialized solution for the severely atrophic lower jaw, where bone loss is so extreme that no conventional option is feasible. The implant system consists of long posts that pass completely through the mandible from the crestal ridge to the inferior cortex beneath the chin, where they are secured by an extraoral baseplate. This provides extremely stable, full-thickness anchorage even in the most severely resorbed lower jaw. Trans-mandibular implants are one of the most technically demanding procedures in implant dentistry, reserved for cases where all other lower jaw implant options have been exhausted. They have been performed since the 1970s and have a well-documented history of success in appropriately selected patients.
